I was a new patient at The Perfect Smile in for Adult Prophylaxis (Cleaning) and Bitewings (Xrays) on May 22, 2021. I wrote on my new patient application Cleaning with X-rays. This is covered 100% by my dental insurance. They provided no reason for not doing Adult Prophylaxis (Cleaning) and Bitewings (Xrays).
Unfortunately, they performed dental services not covered by my dental insurance: 4 quadrants of scaling and root planning, 4 quadrants of gingival irrigation and 6 sites of Arestin, and did not provide adequate reason for not doing an Adult Prophylaxis (Cleaning) and Bitewings (Xrays) on May 22, 2021. Further, I did not receive receipt listing the specific services with charges and I asked for this receipt on May 23, 2021.
Why did The Perfect Smile not do Adult Prophylaxis (Cleaning) and Bitewings (Xrays) on May 22, 2021?
